Use this form to add someone to the prayer list.
YOU MUST HAVE THIER PERMISSION TO DO SO, as this will be displayed on a website that anyone can view.
There is never any cost.
After you fill out this form, we will email you a confirmation. You MUST follow the instructions in the confirmation to complete the process. (Which only involves clicking on a link)
Entries will remain on the website for 1 month. In 1 month, we will email you asking if you want us to continue. If you follow the directions, it will renew the listing for another month. This way we can keep the list fresh.
We will not display your email address on the website. That is only for us to use to validate the entry and to allow for renewals.
If you have any questions or problems, feel free to ask. Also if you change your email address, let us know.
The comments section is optional. You can say anything you like, for example, what type of tumor, or you can leave it blank.
You May use just a last initial if you do not want to use your last name!
